The effectiveness of aerobic physical activity and arginine in complex treatment of obesity in children

Objective. To study the effectiveness of aerobic physical activity and L-arginine within the framework of a complex hospital programme of treatment of obese children.
Patients and methods. The study included 182 obese patients aged 10–17 years, who were randomised to two intervention groups and a control group: group 1 – children who got personalised aerobic physical training as part of a complex treatment for obesity (n = 100); group 2 – children who received 9 g/day L-arginine in addition of aerobic physical training (n = 27); group 3 – children who received a low-calorie diet therapy (n = 55). The dynamics of anthropometric parameters of body composition (bioimpedance analysis), parameters of carbohydrate and lipid metabolism were studied.
Results. Body weight loss in children of the control group (7.05 [5.3; 9.4] kg) was higher than in groups 1 and 2 (4.8 [2.9: 6.3] and 5.6 [3.8; 6.6] kg, respectively, p < 0.01). But comparative analysis of the dynamics of body composition parameters showed a more favourable profile of the change of fat  and fat-free components against the background of aerobic physical training as compared with the standard programme for treatment of obesity. The median of fat mass loss in groups 1, 2 and 3 was 2.3; 2.3 and 2.2 kg, respectively (p > 0.05), lean mass loss – 0.75; 0.9 and 2.5 kg, respectively (p1–3 < 0.001 and p2–3 < 0.001). In group 2 of children who received arginine, stable muscle mass parameters were preserved.
Conclusion. In obese children, aerobic physical activity promotes an active decrease of body fat mass with preservation of fatfree components. Inclusion of arginine in the obesity treatment programme is associated with stabilisation of body muscle mass.
