Iron metabolism in adolescent girls in Helicobacter pylori-associated chronic gastritis

The objective. To study interrelations of chronic Helicobacter pylori infection and iron-deficiency conditions in adolescent girls. Patients and methods. The examination included 94 girls aged 12–18 years who received treatment in a gastroenterological unit.  Instrumental  and  clinical-laboratory  methods  of  examination  were  used:  fibrogastroscopy  with  biopsy,  blood  iron metabolism characteristics. Н.pylori was detected by morphological method and urease test. Statistical methods were used. Results. Iron deficiency among adolescent girls was found in 61.5% of cases, IDA – in 15.4%. Morphologically active gastritis was  diagnosed  in  83.0%  of  girls,  Н.pylori  was  detected  in  75.0%  of  the  examined  patients.  In  the  group  of  infected  girls,  a decrease of hematocrit 39.3% vs 42.5% (p = 0.041) and average red blood cell volume – 97.8 fL vs 83.9 fL (p = 0.033) were noted  as  compared  to  the  group  without  Н.pylori.  In  the  group  of  patients  with  serum  ferritin  levels  below  20  mkg/l,  gastric mucosa colonization with Н.pylori was 100%, in case of the serum ferritin levels above 20 mkg/l colonization was detected in  63.8%,  p  =  0.007.  The  presence  of  mononuclear  infiltration  of  gastric  mucosa  is  associated  with  lower  concentrations of serum iron 16.7 vs 22.8 mkmol/l, p = 0.02. Conclusion. The presence of Н.pylori-associated gastritis in adolescent girls is associated with low concentrations of serum iron and microcytosis.
